Single-Speaker Dictation Template

A structured template format for transcribing single-speaker dictations.

The Single-Speaker Dictation Template is designed for situations where one person is speaking, such as witness statements, adjuster notes, medical assessments, or judicial dictations. It ensures that every section of the dictation is organized, easy to follow, and properly formatted for legal and professional use.

Why Use This Template?

  • Consistency - Every dictation follows the same headings and structure.

  • Clarity - Organizes long monologues into clear sections (e.g., Background, Incident, Repairs).

  • Compliance - Mirrors the formatting standards expected in legal and insurance contexts.

  • Efficiency - Saves time by providing placeholders where dictation is inserted directly.

Key Features of the Template

  • Header Section:
    Includes placeholders for:

    • Case Name

    • Claimant and Insured Parties

    • Claim Number

    • Date of Loss

    • File Number

  • Pre-Defined Headings:
Sections are already broken out to capture dictation logically, including:

    • Introduction

    • Background

    • The Incident

    • Repairs

    • Contents & Unsalvageable Contents

    • Loss of Income

    • Summary

  • Transcript Placeholders: Each section contains a marker, making it clear where dictated text should be inserted.

  • Closing & Signature: Standardized “Yours truly” block with placeholders for name and professional details.

How to Use the Single-Speaker Dictation Template

  1. Open the Template: Save a clean copy before starting a new dictation to avoid overwriting your master file.

  2. Fill Out the Header: Complete the case details (names, claim number, date of loss, file number).

  3. Insert Dictation into Sections: Replace each placeholder with the dictated content. For example:

    • Under Background, insert the dictated case history.

    • Under Repairs, insert the dictated description of remedial work.

  4. Keep Formatting Consistent: Maintain the pre-set fonts, spacing, and headings so the document remains court-ready.

  5. Finalize with Signature: Replace “FIRSTNAME LASTNAME” with your own name, title, and contact details.
